Advance care planning (ACP) is the term used to describe the process of discussing and planning ahead between a patient, their family and the professional about the patient’s future wishes and priorities for care, in anticipation of some deterioration in their condition. ACP offers patient’s the opportunity to plan their future care and support, including medical treatment, while they have the capacity to do so.
